It defines hospice as a special kind of care for patients and families facing a life-limiting illness, with a focus on palliative rather than curative treatment. It describes the admission process as simple and efficient, involving a team that includes physicians, RN case managers, social workers, chaplains, and volunteers. Additionally, it notes the provision of home health aides and on-call services, as well as drugs and supplies/equipment necessary for hospice care. The hospice care provided is available to patients of any age, religion, race, or illness and is covered under Medicare, Medicaid, and most private insurance plans. There’s also an image showing a hospice caregiver with an elderly patient, reinforcing the compassionate care aspect of the service.
It emphasizes simplicity and efficiency, outlining that admitting a loved one into the hospice program involves a straightforward procedure. This process includes coordination with the patient’s physician, hospital discharge planner, social worker, family member, or caregiver. The statement highlights the ease of the process, requiring just one call to the hospice office, which is available 24 hours on-call. A hospice staff member will arrange to meet with the patient’s family to explain the hospice benefits. Once eligibility is determined, all requested hospice services will be provided immediately.
